The Steward Center for Palliative Care is partnering with Landmark Specialty Hospital to provide palliative care within Landmark’s long-term, acute care hospital.
“I am very excited about our partnership with Landmark as it improves the quality of care provided for seriously ill patients in this community. For example, it allows patients to have successful outcomes after rehab, after being on a ventilator or after having had long-term antibiotic therapy,” says Laura Farless, MD, FAAHPM, physician with The Steward Center for Palliative Care. “We follow the patient’s care during hospital stays, and after discharge we can provide outpatient services for continued help with pain management and other symptoms. Our team will also assist in transitioning patients to home health care or hospice care when needed.”
A program of Hospice Savannah, Inc., the Steward Cater for Palliative Care has many valuable partnerships to help assure that everyone in our community has access to quality palliative care. Outpatient clinics are housed in the Lewis Cancer & Research Pavilion at St. Joseph’s/Candler, at the Anderson Cancer Institute at Memorial University Medical Center, and at St. Mary’s Health Center. Patients are also seen throughout Memorial, St. Joseph’s and Candler hospitals.
